精神病儿童个案/小迪克——克莱因
引言:拉康讨论班一第六节。这一节的前一部分,拉康批评安娜•弗洛伊德所有的事情都开始于教育或对自我的说服,质疑她的强大自我与虚弱自我的分类,认为克莱因的方法是完全相反的起点,更接近弗洛伊德式发现的核心,DICK是梅兰妮•克莱因1930年的一篇文章《在自我发展中象征(符号)形成的作用》中的一个个案。
在这篇文章中,她的主要论点建立在这样一个假设:存在一个施虐被激活的心理发展的早期阶段,在这个阶段中吞食母亲的乳房(或母亲的)口腔-施虐的欲望迎来了(施虐的)顶峰时期,在这个时期主体的主导目标是让自己拥有母亲身体的内容物以及通过施虐可以指挥的每一种武器来摧毁她。同时在这个阶段,形成了对俄狄浦斯冲突的引进。生殖阶段开始行使影响,但是还不显着,因为仍是前生殖冲动持有的领域。我的整个论点取决于俄狄浦斯冲突在施虐占主导的时期已开始的事实。

这是一个四岁的小男孩,名叫迪克,他词汇贫乏和智力匮乏,相当于15-18个月大小的孩子的水平。对现实的适应性以及与自己周围环境的情感关联几乎完全缺乏。这个孩子,基本上没有感情,他对母亲和护士的在场或缺席漠不关心。一开始,他几乎不显示出焦虑,且以一个异常小的程度表现。除了有了一个特别的兴趣外,他几乎没有兴趣,不玩耍,没有与他的环境接触。在大多数情况下,他只是用一个毫无意义的方式将声音串在一起,他不断重复一些噪音。当他的确说话时,他通常错误地使用自己微薄的词汇。他不仅是无法使自己被理解:他也不希望这样做。人们可以清楚地感觉到迪克抗拒他的母亲,这个态度通过他经常做与对他被期望的事相反的事表达出来。例如,如果母亲成功的让他跟着自己学说不同的词,她常常整个地改变这些词,尽管在其他的时候他能完美地发出这些词的音。或者,有时他会正确的重复词,但会不断的重复它们在,以机械的方式,直到他周围每个人都恶心或者厌倦了。 这些行为模式与神经症儿童不同。神经症儿童以一种怀疑(不信任)的方式表达对立(对抗),并且当他表达服从(即使伴随过量的焦虑)的时候,他这样做伴随着一定的理解和对所牵扯到的食物或人的某种参照。但是迪克的对抗与服从既缺少感情也缺少理解。当他伤害自己的时候,他显示了巨大的对疼痛的麻木,一点也感觉不到对通常的小孩来说被安慰和抚爱欲望。他身体的笨拙,也相当显着。
他历史的细节。在他是一个吃奶的婴儿的时候,他经历了一个非常不令人满意且失常(紊乱)的时期,因为他的母亲持持了几周不成功的哺乳他的尝试,而且他差点饿死。 然后就求助于人工食物。最后,在他7周大时给他找了个奶妈,但那时他并没有因母乳而茁壮成长。他患有消化不良、脱肛、之后是痔疮。无疑,他的发展受到影响,尽管他受到照料,但是并没有得到过多的真正的爱,他母亲对他的感情从一开始就是冷的。

他在正常年龄学会了走路,但在训练他控制自己的排泄功能时有一个困难。 新奶妈的影响下,他更容易地获得了清洁的习惯。

我从迪克的分析发现,在他的发展中不寻常的抑制作用的原因是那些最早的阶段的失败。在迪克这有一个完全的构成性的忍受焦虑的自我的无能。生殖很早就开始发挥它的作用; 这导致了一个早熟,夸大了对被攻击客体的认同并且促成一个对等的早产的对施虐的防御。 自我停止了发展幻想生活并且停止建立与现实的关系。在一个非常虚弱的开端后,这个孩子的符号形成停顿了。早期的尝试在一个兴趣中留下了它们的印记,这个与现实无关并与之隔离的兴趣不能构成进一步升华的基础。这个孩子对他周围大多数客体和玩物漠不关心,而且甚至不能把握它们的用途和意义。 但是他对火车、车站、门把手、门以及开门、关门感兴趣。
对这些东西和动作的兴趣只有一种来源:确实与阴茎进入母亲身体有关。门与锁代表了进出他母亲身体的方式,门把手代表了他父亲和他自己的阴茎。因此,造成符号形成停顿的东西是对他自已进入母亲身体后将会对他做的事情的害怕(特别是父亲)。而且,他的对破坏冲动的防御证明是对他发展的基本的妨碍。
他是绝对无法做出任何攻击行为,并且无能的基础在非常早的时期是通过他拒绝咬食物清楚地表示出来的。在4岁,他不能持剪刀,刀或工具,并在他所有动作明显笨拙。对指向母亲身体的和其内容的施虐冲动的防御导致幻想的停止和符号形成的停顿。迪克的进一步发展结果是悲伤,因为他不能把对母亲身体的施虐关系引入幻想。
分析中的困难不是有缺陷的言语能力。而是在他头脑里不存在与客体的感情的或符号的关系,偶然的与客体有关的行为也没有染上幻想的颜色,因此,不可能认为它们具有符号表象的特征。他的对周围环境缺乏兴趣以及使得它们与自己的头脑产生联系的困难仅仅是缺乏与物的符号关系的结果。分析必须以此开始,即与他建立交流(接触)的首要的障碍。
第一次分析,我只给他看我放在那儿的玩具,他不带任何兴趣的看着它们。我拿出一个大货车把它放在一个小火车旁边,并且把它们称作“爸爸火车”和“迪克火车”。于是他拿起我称作迪克的火车,让它驶向窗户并且说车站。我解释:“车站是妈妈,迪克将进入妈妈。”
他丢下火车跑向这个房间的内门和外门之间,把自己关在里面,说“黑暗” 然后再立刻跑出来。他经历了几次这个行为。我向他解释:“妈妈里面是黑暗的,迪克在妈妈的黑暗”。其间,他再次拿起火车,立刻跑到两门之间的空间。当我说他将进入妈妈黑暗时,他以询问的方式说了两次:“奶妈?”我回答:“奶妈马上会过来”,他重复的并且之后相当正确的使用的这些词留在了他头脑中。
第二次来,他行动如上次,但他直接跑到黑暗的门厅,他把迪克火车放在那儿,并坚持要求它待在那儿。他不断问:“奶妈来?”
第三小时的分析,他以同样的方式行动。除了跑到门之间与门厅,他还跑到柜子后面。在那儿他被焦虑捕捉,第一次他喊我过去。通过他重复呼唤(找)奶妈的方式,担忧(害怕)现在是很明显了。这一次结束,他以相当不同寻常的快乐迎接奶妈。我们可以看到焦虑出现的同时浮现出了首先是对我之后是对奶妈的依赖感,他开始对我用来抚慰他的词感兴趣,而且与通常的行为不同,他重复并且记住了它们。 这一次,他也第一次的带着兴趣的看着玩具,而且明显有一个攻击的倾向。他指着一个小的煤车说:“切/剪”。我给他一把剪刀,他试图抓住一小块代表煤的黑色木头,但是他拿不住剪刀。
对他看了我一眼的回应,我从车中切出木片,于是,他把被损坏的车及其内容投入到一个抽屉里,说,“走了(离开了)”。我告诉他这意味着迪克被从他母亲那切出来。然后,他跑到两门之间的空间里,用他的指甲略微的抓门,因此,这显示了将这个他正袭击的空间与卡车及他母亲的身体等同。他立刻从门之间的地方跑回来,找到橱柜并爬进去。
下一次分析开始,当奶妈离开时他哭了,但是很快他就平静下来。这次,他回避两扇门之间的空间、橱柜及角落,让自己与玩具产生关系,仔细检查它们,并且伴随曙光般的好奇心。他偶然发现上次被他毁坏的车与内容物,他很快将两者推到一边,并用其它的玩具将它们盖起来。在我解释被毁坏的车代表他母亲之后,他把车与煤再次拿出来,并把它们拿到两门之间。随着分析的进展变得很清晰:通过将它们扔出房间,他指出一个驱除,既是被毁坏的客体又是他自己的施虐(或由赋予的意义),他的施虐以这种方式被投射到外部世界。迪克也发现了洗澡盆象征着他母亲的身体,并且极端害怕被水弄湿。他焦虑地擦去他手上和我手上的水,当他尿尿的时候也显示了同样的焦虑。尿对他来说代表着有害的和危险的东西。在迪克的幻想中变得很清楚了,尿和阴茎代表了袭击母亲身体的客体,也被感受为伤害自己的来源。这些幻想促成他对母亲身体内容物的害怕,尤其害怕被幻想为在母亲子宫中的父亲的阴茎。我们认识到父亲的阴茎和多种形式的对它攻击的增长的感觉,吃掉并毁掉它的欲望变得尤其突出。例如,一次,迪克举起一个玩具男人到嘴巴,咬牙并说“茶爸爸”实际意味着“吃爸爸”。然后他要了一杯水。这个父亲阴茎的投射证明是与对它的害怕联系在一起的,如同原初的、施加惩罚的超我并且害怕被如此劫掠的母亲的惩罚,即害怕外部的被投射的客体。 在他的发展中一个决定性因素是生殖期过早地变得活跃。伴随这些表象而来的不仅有焦虑还有自责、怜悯及他必须被赔偿损失的感觉。 因此,他继续把小男人玩具放到我膝上或我手中,继续将所有东西都放回抽屉里,等等。早期的对源于生殖水平的反应的操作是一个早熟的自我发展的结果,但是进一步的自我发展被它抑制。这个早期的对客体的认同还不能被带进与现实的关系。例如,一次当迪克看到我膝上的铅笔削时,他说:“可怜的克莱因夫人”。但是在一个类似的情况下,他用这种同样的方式说:“可怜的窗帘”。伴随着他忍受焦虑的无能,这个早熟的感情移入(直觉)在他对所有破坏冲动防卫中是一个决定性因素。迪克将自己从现实中切除,并通过逃向黑暗、空、模糊的子宫中带来了他幻想生活的停顿。他因此成功的不仅从外部世界不同的客体中撤回自己的注意力,这些客体代表着他母亲子宫的内容物,也从父亲的阴茎、孩子那撤回注意力。他自己的阴茎作为施虐的器官以及他自己的排泄物被放弃或被否定,因为是危险的和有攻击性的。
在对迪克的分析中对我来说能够接近他的无意识,通过他所展示出来的幻想生活与符号形成的雏形(一点点东西)。结果是减少了他潜在的焦虑,因此一定量的焦虑就能够变得明白。但是这意味着对焦虑的重新工作将开始,通过建立对物与客体符号关系的方式 ,并且同时他的攻击冲动启动了。 每一个前进都伴随着一些新的焦虑的释放并且引导他离开一些他与之建立了情感关系的和成为焦虑客体的东西。当他离开这些,他转向新客体,他的攻击冲动被指向这些新的情感关系。 例如,有时迪克避开橱柜,但是彻底的研究澡盆和点暖气。然后他将自己的兴趣转向新的东西,或者再次转向他已熟悉的更早被放弃的东西。他再次让自己专心于橱柜,但是这次他的兴趣伴随着更大的积极性与好奇心以及更强的各种攻击的趋势。 他用勺子敲这个橱柜、抓它、用一个刀子砍它并且用水洒到它上面。然后他活泼的研究门的合页、锁等等,爬到橱柜里面并且问不同的部分称作什么。因此,当他的兴趣发展了,他同时扩大了他的词汇,因为他现在开始不仅对物感兴趣也对他们的名称感兴趣。这些他之前听到和未理会的词,现在他记住并且正确使用了。
伴随兴趣的发展和对我逐渐增强的转移,到目前为止缺乏的客体关系开始出现了。在这几个月中,他对母亲和奶妈的态度变得正常和有感情。现在他欲望她们出现,想让他们注意他,她们离开时他感到痛苦。和他爸爸在一起的关系逐渐显示出正常的俄狄浦斯态度,并且大体上对客体有逐渐增长的稳固的关系。之前缺乏的让自己被别人理解的欲望现在充分发展。迪克通过仍然较少但在增加中的词汇让自己被别人理解,这些词是他勤奋的努力增加的。迄今他度过了他的六个月的分析,而且他的发展证明了一个良好的展望,这个发展在这一时期在所有基本点上展开。一些在这个个案中产生的特别问题证明可以解决。已经可能借助不少的词与他交流,(通过解释)能够激活孩子内有的焦虑,在这个孩子那里兴趣与感情是完全缺乏的,并且还可以逐渐的解决和控制被释放出来的焦虑。我强调在迪克个案中我修改了我通常的技术这个事实。通常,我不解释材料,直到材料以多种表象找到表达。然而,在这个个案中,表征材料的能力几乎完全丧失,迪克行为中的表象是相对模糊不清的。通过这种方式接近他的无意识,我成功的激活了焦虑与其他感情。之后,表象变得更加丰富,而且不久我获得了这个分析的更牢固的基础,因此能逐渐地转向我通常对小孩子分析中用的技术。

她完全残忍的将象征意义推向他,小迪克。她立刻开始大规模地用解释击中他。她很粗暴的用俄狄浦斯神话的语言表达击中他,对任何读者来说这种表达都是令人不愉快的:你是小火车,你想和你妈妈做爱。

争论中的问题在根源——冲动的根源处是存在(主体)的最初关系。因为每次这些来自主体的最初世界——一个还没有被组织为真正的人类世界、可交流的现实界域——的排出物(驱逐)产生(发生)时,所以,每次一种新类型的认同产生。这是不能忍受的东西,而且焦虑同时产生。
焦虑不是一种主体为了构建客体而分配出去的能量,克莱因的文本不包含任何可能导致这种方式阅读的句子的措词。焦虑总是被定义为突然显现的,如同升起。焦虑总是对应于每一个客体关系的一个认同模式的信号。讨论中的认同先于自我认同。但是,即使当后者能够被实现时,每一个新的主体的再认同会引起焦虑的升起,——在成为一个诱惑一个眩晕一个主体的丧失的意义上的焦虑——主体再发现自己在一个极其原始的水平上。焦虑是一个隐含意义,一个信号,如同弗洛伊德总是很清楚的构想的,一个量,一个主体的染色。
现在这个焦虑正是不产生自考虑中的主体的东西。迪克甚至不能投入最初的认同,这个认同已经是一个符号化的草图。悖论地说,他对峙于现实,生活在现实中。在克莱因的办公室,对他来说没有他者没有自我,只有一个纯粹和简单的现实。这个两扇门之间的空间是他妈妈的身体。火车与随之而来的所有东西,是某种东西,但是某种既不能命名也没有被命名的东西。

第一次分析,我只给他看我放在那儿的玩具,他不带任何兴趣的看着它们。我拿出一个大货车把它放在一个小火车旁边,并且把它们称作“爸爸火车”和“迪克火车”。于是他拿起我称作迪克的火车,让它驶向窗户并且说车站。我解释:“车站是妈妈,迪克将进入妈妈。”

The Importance of Symbol-Formation in the Development of the Ego

My argument in this paper is based on the assumption that there is an early stage of mental development at which sadism becomes active at all the various sources of libidinal pleasure. In my experience sadism reaches its zenith in this phase, which is ushered in by the oral-sadistic desire to devour the mother's breast (or the mother herself) and passes away with the earlier anal stage. At the period of which I am speaking, the subject's dominant aim is to possess himself of the contents of the mother's body and to destroy her by means of every weapon which sadism can command. At the same time this phase forms the introduction to the Oedipus conflict. The genital is beginning to exercise an influence, but this is as yet not evident, for the pregenital impulses hold the field. My whole argument depends on the fact that the Oedipus conflict begins at a period when sadism predominates.
The child expects to find within the mother (a) the father's penis, (b) excrement, and (c) children, and these things it equates with edible substances. According to the child's earliest phantasies (or 'sexual theories') of parental coitus, the father's penis (or his whole body) becomes incorporated in the mother during the act. Thus the child's sadistic attacks have for their object both father and mother, who are in phantasy bitten, torn, cut or stamped to bits. The attacks give rise to anxiety lest the subject should be punished by the united parents, and this anxiety also becomes internalized in consequence of the oral-sadistic introjection of the objects and is thus already directed towards the budding super-ego. I have found these anxiety-situations of the early phases of mental development to be the most profound and overwhelming. It is my experience that in the phantasied attack on the mother's body a considerable part is played by the urethral and anal sadism which is very soon added to the oral and muscular sadism. In phantasy the excreta are transformed into dangerous weapons: wetting

is regarded as cutting, stabbing, burning, drowning, while the f鎐al mass is equated with weapons and missiles. At a later stage of the phase which I have described, these violent modes of attack give place to hidden assaults by the most refined methods which sadism can devise, and the excreta are equated with poisonous substances.
The excess of sadism gives rise to anxiety and sets in motion the ego's earliest modes of defence. Freud writes3: 'It may well be that, before ego and id have become sharply differentiated and before a super-ego has been developed, the mental apparatus employs different modes of defence from those which it practises after these levels of organization have been reached'. According to what I have found in analysis the earliest defence set up by the ego has reference to two sources of danger: the subject's own sadism and the object which is attacked. This defence, in conformity with the degree of the sadism, is of a violent character and differs fundamentally from the later mechanism of repression. In relation to the subject's own sadism the defence implies expulsion, whereas in relation to the object it implies destruction. The sadism becomes a source of danger because it offers an occasion for the liberation of anxiety and also because the weapons employed to destroy the object are felt by the subject to be levelled at his own person also. The object of the attack becomes a source of danger because the subject fears similar, retaliatory attacks from it. Thus, the wholly undeveloped ego is faced with a task which at this stage is quite beyond it梩he task of mastering the severest anxiety.
Ferenczi holds that identification, the forerunner of symbolism, arises out of the baby's endeavour to re-discover in every object his own organs and their functioning. In Jones' view the pleasure-principle makes it possible for two quite different things to be equated because of a similarity marked by pleasure or interest. Some years ago I wrote a paper, based on these statements, in which I drew the conclusion that symbolism is the foundation of all sublimation and of every talent, since it is by way of symbolic equation that things, activities and interests become the subject of libidinal phantasies.
I can now add to what I said then4 and state that, side by side with the libidinal interest, it is the anxiety arising in the phase that I have described which sets going the mechanism of identification. Since

the child desires to destroy the organs (penis, vagina, breast) which stand for the objects, he conceives a dread of the latter. This anxiety contributes to make him equate the organs in question with other things; owing to this equation these in their turn become objects of anxiety, and so he is impelled constantly to make other and new equations, which form the basis of his interest in the new objects and of symbolism.
Thus, not only does symbolism come to be the foundation of all phantasy and sublimation but, more than that, upon it is built up the subject's relation to the outside world and to reality in general. I pointed out that the object of sadism at its zenith, and of the epistemophilic impulse arising and co-existing with sadism, is the mother's body with its phantasied contents. The sadistic phantasies directed against the inside of her body constitute the first and basic relation to the outside world and to reality. Upon the degree of success with which the subject passes through this phase will depend the extent to which he can subsequently acquire an external world corresponding to reality. We see then that the child's earliest reality is wholly phantastic; he is surrounded with objects of anxiety, and in this respect excrement, organs, objects, things animate and inanimate are to begin with equivalent to one another. As the ego develops, a true relation to reality is gradually established out of this unreal reality. Thus, the development of the ego and the relation to reality depend on the degree of the ego's capacity at a very early period to tolerate the pressure of the earliest anxiety-situations. And, as usual, it is a question of a certain optimum amount of the factors concerned. A sufficient quantity of anxiety is the necessary basis for an abundance of symbol-formation and of phantasy; an adequate capacity on the part of the ego to tolerate anxiety is necessary if it is to be satisfactorily worked over and if this basic phase is to have a favourable issue and the development of the ego to be successful.
I have arrived at these conclusions from my general analytical experience, but they are confirmed in a remarkably striking way by a case in which there was an unusual inhibition of ego-development.
This case, of which I will now give some details, is that of a four-year-old boy who, as regards the poverty of his vocabulary and of his intellectual attainments, was on the level of a child of about fifteen or eighteen months. Adaptation to reality and emotional relations to his environment were almost entirely lacking. This child, Dick, was largely devoid of affects, and he was indifferent to the presence or

absence of mother and nurse. From the very beginning he had only rarely displayed anxiety, and that in an abnormally small degree. With the exception of one particular interest, to which I will return presently, he had almost no interests, did not play, and had no contact with his environment. For the most part he simply strung sounds together in a meaningless way, and certain noises he constantly repeated. When he did speak he generally used his meagre vocabulary incorrectly. But it was not only that he was unable to make himself intelligible: he had no wish to do so. More than that, one could clearly perceive that Dick was antagonistic to his mother, an attitude which expressed itself in the fact that he often did the very opposite of what was expected of him. For instance, if she succeeded in getting him to say different words after her, he often entirely altered them, though at other times he could pronounce the same words perfectly. Again, sometimes he would repeat the words correctly, but would go on repeating them in an incessant, mechanical way until everyone round him was sick and tired of them. Both these modes of behaviour are different from that of a neurotic child. When the neurotic child expresses opposition in the form of defiance and when he expresses obedience (even accompanied by an excess of anxiety), he does so with a certain understanding and some sort of reference to the thing or person concerned. But Dick's opposition and obedience lacked both affect and understanding. Then too, when he hurt himself, he displayed very considerable insensibility to pain and felt nothing of the desire, so universal with little children, to be comforted and petted. His physical awkwardness, also, was quite remarkable. He could not grip knives or scissors, but it was noteworthy that he could handle quite normally the spoon with which he ate.
The impression his first visit left on me was that his behaviour was quite different from that which we observe in neurotic children. He had let his nurse go without manifesting any emotion, and had followed me into the room with complete indifference. There he ran to and fro in an aimless, purposeless way, and several times he also ran round me, just as if I were a piece of furniture, but he showed no interest in any of the objects in the room. His movements as he ran to and fro seemed to be without co-ordination. The expression of his eyes and face was fixed, far-away and lacking in interest. Compare once more the behaviour of children with severe neuroses. I have in mind children who, without actually having an anxiety-attack, would on their first visit to me withdraw shyly and stiffly into a corner or sit motionless

before the little table with toys on it or, without playing, lift up one object or another, only to put it down again. In all these modes of behaviour the great latent anxiety is unmistakable. The corner or the little table is a place of refuge from me. But Dick's behaviour had no meaning or purpose, nor was any affect or anxiety associated with it.
I will now give some details of his previous history. He had had an exceptionally unsatisfactory and disturbed time as a sucking infant, for his mother kept up for some weeks a fruitless attempt to nurse him, and he nearly died of starvation. Artificial foods were then resorted to. At last, when he was seven weeks old, a wet-nurse was found for him, but by then he did not thrive on breast-feeding. He suffered from indigestion, prolapsus ani and, later, from h鎚orrhoids. Undoubtedly his development was affected by the fact that, though he had every care, no real love was lavished on him, his mother's feeling for him being from the very beginning cold.5
As, moreover, neither his father nor his nurse gave him any tenderness, Dick grew up in an environment unusually poor in love. When he was two years old he had a new nurse, who was skilful and affectionate, and, shortly afterwards, he was for a considerable time with his grandmother, who was very loving to him. The influence of these changes was observable in his development. He had learnt to walk at about the normal age, but there was a difficulty in training him to control his excretory functions. Under the new nurse's influence he acquired habits of cleanliness much more readily. At the age of about three he had mastered them, and on this point he actually showed a certain amount of ambition and apprehensiveness. In one other respect he showed himself in his fourth year sensitive to blame. The nurse had found out that he practised masturbation and had told him it was 'naughty' and he must not do it. This prohibition clearly gave rise to apprehension and to a sense of guilt. Moreover, in his fourth year Dick did in general make a greater attempt at adaptation, but principally in relation to external things, especially to the mechanical learning of a number of new words. From his earliest days the question of feeding had been abnormally difficult. When he had the wet-nurse he showed no desire at all to suck, and this disinclination persisted. Next, he would not drink from a bottle. When the time came for him to have more solid food, he refused to bite it up and absolutely

rejected everything that was not of the consistency of pap; even this he had almost to be forced to take. Another good effect of the new nurse's influence was an improvement in Dick's willingness to eat, but even so, the main difficulties persisted.6 Thus, although the kindly nurse had made a difference to his development in certain respects, the fundamental defects remained untouched. With her, as with everyone else, Dick had failed to establish friendly contact. Thus neither her tenderness nor that of his grandmother had succeeded in setting in train the lacking object-relation.
I found from Dick's analysis that the reason for the unusual inhibition in his development was the failure of those earliest steps of which I spoke at the beginning of this paper. In Dick there was a complete and apparently constitutional incapacity of the ego to tolerate anxiety. The genital had begun to play its part very early; this caused a premature and exaggerated identification with the object attacked and had contributed to an equally premature defence against sadism. The ego had ceased to develop phantasy-life and to establish a relation with reality. After a feeble beginning, symbol-formation in this child had come to a standstill. The early attempts had left their mark in one interest, which, isolated and unrelated to reality, could not form the basis for further sublimations. The child was indifferent to most of the objects and playthings around him, and did not even grasp their purpose or meaning. But he was interested in trains and stations and also in door-handles, doors and the opening and shutting of them.
The interest in these things and actions had a single source: it really had to do with the penetration of the penis into the mother's body. Doors and locks stood for the ways in and out of her body, while the door-handles represented the father's penis and his own. Thus what had brought symbol-formation to a standstill was the dread of what would be done to him (particularly by the father's penis) after he had penetrated into the mother's body. Moreover, his defences against his destructive impulses proved to be a fundamental impediment to his development. He was absolutely incapable of any act of aggression, and the basis of this incapacity was clearly indicated at a very early period in his refusal to bite up food. At four years old he could not hold scissors, knives or tools, and was remarkably clumsy in all his movements. The defence against the sadistic

impulses directed against the mother's body and its contents梚mpulses connected with phastasies of coitus梙ad resulted in the cessation of the phantasies and the standstill of symbol-formation. Dick's further development had come to grief because he could not bring into phantasy the sadistic relation to the mother's body.
The peculiar difficulty I had to contend with in the analysis was not his defective capacity for speech. In the play-technique, which follows the child's symbolic representations and gives access to his anxiety and sense of guilt, we can, to a great extent, dispense with verbal associations. But this technique is not restricted to an analysis of the child's play. Our material can be derived (as it has to be in the case of children inhibited in play) from the symbolism revealed in details of his general behaviour.7 But in Dick symbolism had not developed. This was partly because of the lack of any affective relation to the things around him, to which he was almost entirely indifferent. He had practically no special relations with particular objects, such as we usually find in even severely inhibited children. Since no affective or symbolic relation to them existed in his mind, any chance actions of his in relation to them were not coloured by phantasy, and it was thus impossible to regard them as having the character of symbolic representations. His lack of interest in his environment and the difficulty of making contact with his mind were, as I could perceive from certain points in which his behaviour differed from that of other children, only the effect of his lack of a symbolic relation to things. The analysis, then, had to begin with this, the fundamental, obstacle to establishing contact with him.
The first time Dick came to me, as I said before, he manifested no sort of affect when his nurse handed him over to me. When I showed him the toys I had put ready, he looked at them without the faintest interest. I took a big train and put it beside a smaller one and called them 'Daddy-train' and 'Dick-train'. Thereupon he picked up the train I called 'Dick' and made it roll to the window and said 'Station'. I explained: 'The station is mummy; Dick is going into mummy'. He

After we had analysed this anxiety his attitude towards both these functions was very different and is now almost normal.

left the train, ran into the space between the outer and inner doors of the room, shut himself in, saying 'dark' and ran out again directly. He went through this performance several times. I explained to him: 'It is dark inside mummy. Dick is inside dark mummy'. Meantime he picked up the train again, but soon ran back into the space between the doors. While I was saying that he was going into dark mummy, he said twice in a questioning way: 'Nurse'? I answered: 'Nurse is soon coming', and this he repeated and used the words later quite correctly, retaining them in his mind. The next time he came, he behaved in just the same way. But this time he ran right out of the room into the dark entrance hall. He put the 'Dick' train there too and insisted on its staying there. He kept constantly asking: 'Nurse coming'? In the third analytic hour he behaved in the same way, except that besides running into the hall and between the doors, he also ran behind the chest of drawers. There he was seized with anxiety, and for the first time called me to him. Apprehension was now evident in the way in which he repeatedly asked for his nurse, and, when the hour was over, he greeted her with quite unusual delight. We see that simultaneously with the appearance of anxiety there had emerged a sense of dependence, first on me and then on the nurse, and that at the same time he had begun to be interested in the words I used to soothe him and, contrary to his usual behaviour, had repeated and remembered them. During the third hour, however, he also, for the first time, looked at the toys with interest, in which an aggressive tendency was evident. He pointed to a little coal-cart and said: 'Cut'. I gave him a pair of scissors, and he tried to scratch the little pieces of black wood which represented coal, but he could not hold the scissors. Acting on a glance which he gave me, I cut the pieces of wood out of the cart, whereupon he threw the damaged cart and its contents into a drawer and said, 'Gone'. I told him that this meant that Dick was cutting f鎐es out of his mother. He then ran into the space between the doors and scratched on the doors a little with his nails, thus showing that he identified the space with the cart and both with the mother's body, which he was attacking. He immediately ran back from the space between the doors, found the cupboard and crept into it. At the beginning of the next analytic hour he cried when the nurse left him梐n unusual thing for him to do. But he soon calmed down. This time he avoided the space between the doors, the cupboard and the corner, but concerned himself with the toys, examining them more closely and with obviously dawning curiosity. Whilst

doing this he came across the cart which had been damaged the last time he came and upon its contents. He quickly pushed both aside and covered them with other toys. After I had explained that the damaged cart represented his mother, he fetched it and the little bits of coal out again and took them into the space between the doors. As his analysis progressed it became clear that in thus throwing them out of the room he was indicating an expulsion, both of the damaged object and of his own sadism (or the means employed by it), which was in this manner projected into the external world. Dick had also discovered the wash-basin as symbolizing the mother's body, and he displayed an extraordinary dread of being wetted with water. He anxiously wiped it off his hand and mine, which he had dipped in as well as his own, and immediately afterwards he showed the same anxiety when urinating. Urine and f鎐es represented to him injurious and dangerous substances.8
It became clear that in Dick's phantasy f鎐es, urine and penis stood for objects with which to attack the mother's body, and were therefore felt to be a source of injury to himself as well. These phantasies contributed to his dread of the contents of his mother's body, and especially of his father's penis which he phantasied as being in her womb. We came to recognize the father's penis and a growing feeling of aggression against it in many forms, the desire to eat and destroy it being specially prominent. For example, on one occasion, Dick lifted a little toy man to his mouth, gnashed his teeth and said 'Tea daddy', by which he meant 'Eat daddy'. He then asked for a drink of water. The introjection of the father's penis proved to be associated with the dread both of it, as of a primitive, harm-inflicting super-ego, and of being punished by the mother thus robbed: dread,

that is, of the external and the introjected objects. And at this point there came into prominence the fact which I have already mentioned, and which was a determining factor in his development, namely, that the genital phase had become active in Dick prematurely. This was shown in the circumstance that such representations as I have just spoken of were followed not by anxiety only, but by remorse, pity and a feeling that he must make restitution. Thus he would proceed to place the little toy men on my lap or in my hand, put everything back in the drawer, and so on. The early operation of the reactions originating on the genital level was a result of premature ego-development, but further ego-development was only inhibited by it. This early identification with the object could not as yet be brought into relation with reality. For instance, once when Dick saw some pencil shavings on my lap he said 'Poor Mrs. Klein'. But on a similar occasion he said in just the same way, 'Poor curtain'. Side by side with his incapacity for tolerating anxiety, this premature empathy became a decisive factor in his warding-off of all destructive impulses. Dick cut himself off from reality and brought his phantasy-life to a standstill by taking refuge in the phantasies of a dark, empty, vague womb. He had thus succeeded in withdrawing his attention also from the different objects in the outside world which represented the contents of the mother's womb梩he father's penis, f鎐es, children. His own penis, as the organ of sadism, and his own excreta were to be got rid of (or denied) as being dangerous and aggressive.
It had been possible for me, in Dick's analysis, to gain access to his unconscious by getting into contact with such rudiments of phantasy-life and symbol-formation as he displayed. The result was a diminution of his latent anxiety, so that it was possible for a certain amount of anxiety to become manifest. But this implied that the working-over of this anxiety was beginning by way of the establishment of a symbolic relation to things and objects, and at the same time his epistemophilic and aggressive impulses were set in action. Every advance was followed by the releasing of fresh quantities of anxiety and led to his turning away to some extent from the things with which he had already established an affective relation and which had therefore become objects of anxiety. As he turned away from these he turned towards new objects, and his aggressive and epistemophilic impulses were directed to these new affective relations in their turn. Thus, for instance, for some time Dick altogether avoided the cupboard, but thoroughly investigated the wash-basin and the

electric radiator, which he examined in every detail, again manifesting destructive impulses against these objects. He then transferred his interest from them to fresh things or, again, to things with which he was already familiar and which he had given up earlier. He occupied himself once more with the cupboard, but this time his interest in it was accompanied by a far greater activity and curiosity and a stronger tendency to aggression of all kinds. He beat on it with a spoon, scratched and hacked it with a knife and sprinkled water on it. He then briskly investigated the hinges of the door, the way in which it opened and shut, the lock, etc., climbed up inside the cupboard and asked what the different parts were called. Thus as his interests developed he at the same time enlarged his vocabulary, for he now began to take more and more interest not only in the things themselves but in their names. The words which before he had heard and disregarded he now remembered and applied correctly.
Hand in hand with this development of interests and an increasingly strong transference to myself, the hitherto lacking object-relation has made its appearance. During these months his attitude to his mother and nurse has become affectionate and normal. He now desires their presence, wants them to take notice of him and is distressed when they leave him. With his father, too, his relation shows increasing indications of the normal Oedipus attitude, and there is an increasingly firm relation to objects in gerneral. The desire to make himself intelligible, which was lacking before, is now in full force. Dick tries to make himself understood by means of his still meagre, but growing, vocabulary which he diligently endeavours to enlarge. There are many indications, moreover, that his relation to reality is becoming established.
So far we have spent six months over his analysis, and his development, which has begun to take place at all the fundamental points during this period, justifies a favourable prognosis. Several of the peculiar problems which arose in his case have proved soluble. It has been possible to get into contact with him with the help of quite a few words, to activate anxiety in a child in whom interest and affect were wholly lacking, and it has further been possible gradually to resolve and to regulate the anxiety released. I would emphasize the fact that in Dick's case I have modified my usual technique. In general I do not interpret the material until it has found expression in various representations. In this case, however, where the capacity to represent it was almost entirely lacking, I found myself obliged to make my interpretations on the basis of my general knowledge, the representations in

Dick's behaviour being relatively vague. Finding access in this way to his unconscious, I succeeded in activating anxiety and other affects. The representations then became fuller and I soon acquired a more solid foundation for the analysis, and so was able gradually to pass over to the technique that we generally employ in analysing little children.
I have already described how I succeeded in causing the anxiety to become manifest by diminishing it in its latent state. When it did manifest itself, I was able to resolve part of it by interpretation. At the same time, however, it became possible for it to be worked over in a better way, namely, by its distribution amongst new things and interests; in this manner it became so far mitigated as to be tolerable for the ego. Whether, if the quantities of anxiety are thus regulated, the ego can become capable of tolerating and working over normal quantities, only the further course of the treatment can show. In Dick's case, therefore, it is a question of modifying a fundamental factor in his development by means of analysis.
The only possible thing to do in analysing this child, who could not make himself intelligible and whose ego was not open to influence, was to try to gain access to his unconscious and, by diminishing the unconscious difficulties, to open up a way for the development of the ego. Of course, in Dick's case as in every other, access to the unconscious had to be by way of the ego. Events proved that even this very imperfectly developed ego was adequate for establishing connection with the unconscious. From the theoretical point of view I think it is important to note that, even in so extreme a case of defective ego-development, it was possible to develop both ego and libido simply by analysing the unconscious conflicts, without bringing any educational influence to bear upon the ego. It seems plain that, if even the imperfectly developed ego of a child who had no relation at all with reality can tolerate the removal of repressions by the aid of analysis, without being overwhelmed by the id, we need not fear that in neurotic children (i.e. in very much less extreme cases) the ego might succumb to the id. It is also noteworthy that, whereas the educational influence exercised by those about him previously glided off Dick without any effect, now, when owing to analysis his ego is developing, he is increasingly amenable to such influence, which can keep pace with the instinctual impulses mobilized by analysis and quite suffices to deal with them.
There still remains the question of diagnosis. Dr. Forsyth diagnosed the case as one of dementia pr鎐ox and thought it a suitable one

in which to attempt analysis. His diagnosis would seem to be corroborated by the fact that the clinical picture agreed in many important points with that of advanced dementia pr鎐ox in adults. To summarize it once again: it was characterized by an almost complete absence of affect and anxiety, a very considerable degree of withdrawal from reality and of inaccessibility, a lack of emotional rapport, negativistic behaviour alternating with signs of automatic obedience, indifference to pain, perseveration梐ll symptoms which are characteristic of dementia pr鎐ox. Moreover, this diagnosis is further corroborated by the fact that the presence of any organic disease can be certainly excluded, firstly, because Dr. Forsyth's examination revealed none and, secondly, because the case has proved amenable to psychological treatment. The analysis showed me that the idea of a psycho-neurosis could be definitely dismissed.
Against the diagnosis of dementia pr鎐ox is the fact that the essential feature of Dick's

Importance de la formation du symbole dans la formation de l'ego.
Il m'a paru difficile de le résumer. Il a fallu juxtaposer des notions que j'ai eu du mal à raccorder. Je ne vais pas en répéter tout à fait le plan.
Je vais commen¬cer par exposer le cas de l'enfant
en question, un garçon de quatre ans, pour que
la question posée apparaisse plus concrètement.
Je vous dirai ensuite :
- ce qui est l'introduction du chapitre,
- comment elle conçoit en général le passage d'un cer¬tain stade à un autre,
- et ensuite comment elle applique ces considérations,
- com¬ment elle comprend à travers ces considérations le cas de l'enfant, l'amorce du traitement qu'elle nous donne, sa compréhension du traitement.
Pour finir, je dirai comment j'ai régi là-devant.
Il s'agit d'un enfant, garçon de quatre ans, qui a les caractéristiques suivantes :
- il a un niveau général de développement qui correspond, dit-elle, à 15 à 18 mois,
- un vocabulaire très limité, et plus que limité, incorrect : il déforme les mots et les emploie mal à propos la plupart du temps, alors qu'à d'autres moments on se rend compte qu'il en connaît le sens,
- elle insiste sur le fait le plus frappant cet enfant n'a pas le désir de se faire comprendre, il ne cherche pas à communi¬quer,
- elle trouve qu'il n'a aucune adaptation au réel et aucune relation émo¬tionnelle, il est dépourvu d'affect dans toutes les circonstances de la vie quotidienne : il ne réagit pas, ni à la présence ni à l'absence de sa mère ou de sa nurse,
- il ne montre aucune anxiété dans aucune circonstance,
- il ne joue pas. Ses seules activités plus ou moins ludiques seraient d'émettre des sons et se com¬plaire dans des sons sans signification, auxquels on ne peut pas donner de signi¬fication, dans des bruits.
- Il est tout à fait insensible à la douleur physique : il ne réagit pas quand il se fait mal.
- Vis-à-vis des adultes, mère, père, nurse, il a deux attitudes tour à tour. Elle explique cela vis-à-vis du vocabulaire : ou bien il s'op¬pose, systématiquement, par exemple quand on veut lui faire répéter des mots, ou il ne les répète pas, ou il les déforme, ou alors, s'il prononce les mots cor¬rectement, il paraît leur enlever leur sens, et il les mouline, il les répète d'une façon stéréotypée, bien que les mots soient corrects, ça ne veut plus rien dire.
- Par ailleurs, il ne recherche jamais aucune caresse de ses proches, de ses parents.
- Et elle finit la description en insistant sur sa maladresse physique. Deux choses : d'une part
il est maladroit en général, et mal coordonné, plus précisément, il se montre incapable de tenir des ciseaux et des couteaux, alors qu'il manipule très bien sa cuiller à table.
Voilà les éléments d'histoire de l'enfant.
À la naissance, la mère n'avait pas de lait suffisamment, et il n'était pas bon. Elle a tenu pourtant à le nourrir pendant sept semaines,
et pendant sept semaines il a dépéri continuellement. Au bout de sept semaines, on lui a proposé une nourrice, mais il a refusé le sein. Ensuite, on lui a proposé le biberon, il l'a refusé.
Quand est arrivé l'âge des nourritures solides,
il les a également refusées : il ne veut pas mordre.
Depuis le départ il a eu des troubles digestifs importants, un peu plus tard, des hémorroïdes et un prolapsus anal.
L'enfant est toujours un grand anorexique,
il l'a toujours été, c'est le symp¬tôme qui cédera
le plus difficilement au traitement.
Elle dit que de plus cet enfant n'a jamais été entouré d'amour vrai. Sa mère était anxieuse, peu maternelle. Son père et sa nurse, très indifférents.
À deux ans il a eu un apport affectif positif : une nouvelle nurse…
qui, elle, était aimante et affectueuse
…et une grand-mère avec qui il a eu un contact.
À partir de ce moment, l'enfant a été propre. Il est arrivé à contrôler ses excréments et à le dési¬rer, pour la nurse, semble-t-il, pour lui faire plaisir.
Il a fait ce que Mélanie ΚLEIN appelle tentative d'adaptation. Il est partiellement arrivé, a appris un certain nombre de mots, a augmenté tout d'un coup son vocabulaire, mais a continué à l'employer mécaniquement.
Ιl s'est montré sensible à des interdictions de
la nurse, une seule concernant la masturbation :
elle l'a appelé « méchant garçon », et depuis il ne s'est plus, du moins on ne l'a plus vu se masturber.
Donc une certaine amélioration :
augmentation du vocabulaire, tentative d'adaptation aux objets.
Mais :
- l'anorexie continue.
- Il ne tient toujours pas mieux couteaux et ciseaux, et refuse d'absorber quelque nourriture que ce soit, autre que liquide ou bouillie.
Ce que Mélanie KLEIN souligne en terminant cette description, c'est que mal¬gré ces progrès faits à travers la présence de la nurse, il n'avait aucun contact émotionnel avec la nurse, quelques progrès superficiels, mais le contact émo¬tionnel réel n'existait pas pour autant.
Le problème que se pose Mélanie ΚLEIN en prenant
cet enfant en traitement est simplement :
pourquoi une telle absence de contact avec les êtres humains et avec les choses ? Que se passe-t-il ?
Comment décrire une pareille situation ?
je vous demande maintenant de laisser de côté le cas clinique de l'enfant, qui s'appelle DICK, pour revenir aux considérations théoriques qui forment le début
du chapitre et introduisaient en réalité ce cas.
Ces considérations théoriques, j'ai eu beaucoup de mal à les résumer. Ce qui m'a paru difficile est qu'elle passe continuellement, dans le texte, du plan des contenus, de ce que dit l'enfant, au plan des structures, et sans dire qu'elle y passe.
Et on est toujours perdu entre les deux.
Elle centre ces considérations théoriques autour du problème suivant :
comment un enfant, un être humain peut-il passer
du stade sadique-oral, je vais préciser, au stade suivant,
qui est le stade du symbolisme ?
Le stade sadique-oral, elle le décrit surtout…
et c'est ça qui est difficile
…par le contenu, ce que vous savez tous :
- comment l'enfant désire s'approprier le corps de la mère, le contenu du corps de la mère,
- comment il se fait une équivalence entre le contenu du corps et les excréments, les enfants possibles, le pénis du père dans le corps de la mère,
- comment l'enfant, introjectant tout cela, est ensuite envahi de mauvaises choses, qu'il n'a plus qu'à expulser, pour les réintrojec¬ter de nouveau.
Elle décrit un cercle vicieux…
dont on ne voit pas comment il peut être rompu
…en fait, d'introjections et d'expulsions, et où moi je n'ai pas compris où elle situait l'enfant lui-même. On ne voit que le circuit, mais pas le sujet lui-même.
LACAN - C'est bien justement de cela qu'il s'agit.

Alors, elle se pose la question suivante :
le sujet, l'enfant, est anxieux de quoi ?
Du sadisme qu'il éprouve, qu'il manifeste à l'égard de la mère, du corps de la mère, des contenus du corps de la mère et de ses équivalents.
Il est anxieux de détruire la mère, et aussi de se détruire lui-même en retour, puis¬qu'il introjecte la mère, donc son propre sadisme. Il est anxieux de détruire l'ob¬jet d'amour et de se détruire lui-même.
Χ - C'est bien compliqué, tout ça.

Alors, elle pense que cette anxiété déclenche
le premier mode de défense, que l'on peut décrire,
un mode de défense qu'elle dit fondamentalement différent des mécanismes de dépression ultérieurs.

S'appuyant sur un texte de FREUD, disant qu'à l'origine il doit y avoir quelque chose d'autre que tout ce que nous pouvons insérer dans les structures autour de la notion du moi. Évidemment, puisque nous sommes avant la formation du moi.

Ces mécanismes de répression sont de deux sortes :
- l'ex¬pulsion : rejet à l'extérieur de son propre sadisme,
- et détruire cet objet dange¬reux.

Je ne comprends pas comment elle-même s'y retrouve ?

Là elle introduit la notion de symbolisme, en se référant à FERENCZI et JONES pour qui le symbolisme est le fondement de toute sublimation et de toute action sur le monde extérieur.
Cela, c'est presque le plus important du texte.
Et j'espère avoir bien compris comment elle comprend la notion de symbolisme.
Pour elle, le symbolisme est le fait que le sujet fait une équation, une égalité entre lui corporel, son corps, lui entier, ou les parties de son corps, le corps de la mère, pénis, vagin, enfants, excréments, et des objets externes. Pour elle, le symbolisme est une égalité mise entre le corps propre et les objets externes.
Elle pense alors que c'est en réalisant de pareilles équations multiples…

à propos de l'article où il montre les équivalences dans les structures anales, là où il présente
le grand schéma même dessiné, objectivé, où il montre l'équivalence : enfant = phallus = excréments etc.

- que pour que le moi se développe bien,
- que pour que l'anxiété du sadisme de la phase orale, soit dépassée,
…il faut qu'il y ait une certaine quantité minimum d'anxiété,
faute de quoi le sujet ne la distribuerait pas
sur le monde extérieur, mais qu'il n'en faut pas trop : c'est toute une ques¬tion de quantité optimum, il faut que l'ego puisse la tolérer pour la maîtriser. Elle ne précise pas non plus la maîtrise.
Maintenant elle reprend ses considérations générales à propos du cas de DICK, garçon de quatre ans.

et cela elle ne le justifie pas du tout
…a une capacité constitutionnelle de son moi à tolérer l'anxiété.
Elle pense ensuite…
et elle le jus¬tifie plus loin,
d'une manière que je ne comprends pas
…que les pulsions géni¬tales - proprement génitales - ont été particulièrement précoces chez DICK, et que ces pulsions génitales ont entraîné une moindre tolérance des pulsions sadiques et des défenses accrues contre les pulsions sadiques.
Et alors, dit-elle, DICK n'a pas pu…
parce qu'il avait trop peur de son sadisme, étant trop génital, supportant trop mal ce sadisme
…n'a pas pu faire cette distribution d'anxiété sur les objets du monde extérieur, mais seulement sur deux ou trois objets, qu'elle cite, qui étaient les seules activités ludiques de DICK :
l'intérêt pour les trains, pour les gares, pour les portes,
les trois choses qu'il manipulait.

Peut-être, là, faites-vous quand même une élision,
me semble-t-il, qui est importante si vous la maintenez, de la description que vous avez faite du cas clinique. Il y avait quelque chose d'autre qui est le comportement de DICK chez Mélanie KLEIN.

c'est surtout chez Mélanie KLEIN, que ça a lieu.

Là, c'est ce qu'elle dit avant.
Elle pense que les seules dis¬tributions faites sur l'extérieur, étant donné son anxiété, sont les trains,
les gares, les portes, et que ces trois objets représentent symboliquement :
- le train : le pénis,
- la gare : la mère,
- et les boutons de portes : […]
Et elle pense que la distribution d'anxiété sur les objets s'est arrêtée là.
Cet arrêt de la distribution de l'anxiété sur le monde extérieur constitue le manque de contact de DICK, qui n'est en contact avec rien, parce qu'il n'a pas distribué son anxiété sur l'extérieur.
Et alors, elle dit que c'est sa peur de son sadisme, sa défense contre son sadisme, qui de même le rend incapable de tout acte agressif, comme de mordre, de manipuler les ciseaux et les couteaux.
Et cette même défense l'empêche de traduire en fantasmes tout ce qu'il vit, sa relation sadique au corps de la mère.
Ensuite, elle nous raconte les premières séances d'analyse, très résumées, et l'attitude de DICK pendant les premières séances.
Il est entré chez elle.
Il a quitté sa nurse sans aucune émotion, a erré sans but dans la pièce de traitement, ne s'est intéressé
à aucun objet, a fait des mou¬vements tout à fait incoordonnés, désordonnés, sans signification, une mimique figée, un regard tout à fait perdu et absent, et, dit-elle :
« Comme si je n'étais pas autre chose qu'un meuble, aucune de ses atti¬tudes ne s'adressait à moi. »
Elle insiste sur le fait que DICK avait pour elle une conduite, une attitude dif¬férente des grands névrosés qu'elle traite habituellement. Elle décrit les névro¬sés les plus névrosés, qui se mettent dans un coin ou se cachent. Elle insiste :
« C'est comme s'il avait été dans le vide. »
Et, connaissant son intérêt pour les trains et les gares, et connaissant que c'était son seul investissement anxieux d'objets, elle a pris un grand train qu'elle a mis à côté d'un petit train.
LACAN
Vous êtes sûre qu'il y a ça, qu'elle connaissait ça ? Elle ne connais¬sait rien de pareil, elle lui a foutu le train dans les mains.
X - Si. Elle le connaissait, elle le savait avant.
LACAN - D'ailleurs, ça ne change pas grand-chose.
Marie-Cécile GÉLINIER
Il appelle le grand train « Papa-train », et le petit train « DICK-train ». Il prend le petit DICK-train, le fait rouler jusque sous la fenêtre.
Et déjà elle verbalise :
« La gare, c'est maman. DICK va dans Maman. »
Alors à ce moment-là, l'enfant lâche le train
et court à toutes jambes dans un espace sombre
entre deux portes, dans la double porte de la salle de consultation. Elle va, et il dit :
« Noir ».
Elle verbalise : « C'est noir dans Maman ».
À ce moment-là, l'enfant questionne en disant :
« Nurse »
Mais d'une voix qui, cette fois, avait un sens :
il demande sa nurse, ce qu'il ne faisait jamais.
Elle répond :
« Nurse reviendra bientôt. »
L'enfant répète ces mots, d'une manière intelligente, ils ont un sens qui cor¬respondait à la situation.
Et Mélanie KLEIN remarque qu'à partir de ce jour-là elle pense qu'il a appris ces mots, qu'il s'en est rappelé. Il les a réutilisés par la suite correctement.
À la séance suivante, il entre dans la salle de traitement
tout à fait de la même façon, mais cette fois-ci,
il va dans le coin noir et il met le DICK-train dans
ce coin noir et veut qu'il y reste. Et il répète :
« Est-ce que nurse vient ? »
À la troisième fois, il va aussi dans le coin noir derrière la commode, et là il est très anxieux, et pour la première fois il appelle l'analyste auprès de lui.
L'analyste remarque que le fait qu'il investisse un plus grand nombre d'ob¬jets, la commode, le coin noir de la porte, en plus des trains correspond avec
un sentiment de dépendance vis-à-vis de la nurse
et d'elle-même, puisqu'il réclame leur présence,
avec la première anxiété exprimée, manifestée,
qui est d'appeler la nurse.
Ensuite, à la quatrième séance, il désigne un petit wagon à charbon en disant :
« cut ! », couper.
L'analyste lui donne une paire de ciseaux.
Il essaie de malmener le contenu du petit wagon
à charbon, mais n'y arrive pas.
L'analyste prend les ciseaux à son compte, le sadisme à son compte, et fait elle-même le dépeçage du contenu du wagon à charbon.
Une fois que c'est fini, l'enfant prend le tout,
le jette dans un tiroir, et dit :
« Parti ! ».
Alors elle verbalise :
« DICK voulait enlever, couper, les excréments du corps de sa mère. »
La fois suivante, il revoit le même wagon à charbon, le cache, anxieux, sous les autres jouets.
Elle lui répète son anxiété.
Il ressort le wagon à charbon.
Elle lui dit qu'il a voulu prendre et couper ce qu'il y avait dans le corps de sa mère.
À ce moment-là l'enfant de nouveau court dans l'espace sombre entre les deux pièces et - dit Mélanie KLEIN - il montre bien que l'espace sombre entre les deux pièces représentait aussi le corps
de sa mère, car quand je lui ai dit qu'« il voulait
attaquer l'intérieur du corps de sa mère », avec ses ongles il gratte, c'est-à-dire répète la même chose :
il abîme l'intérieur du corps de sa mère.
Il n'y a rien d'autre dans ce qu'elle donne comme contenu, sinon que ces mêmes jeux sadiques se continuent. Il distribue de plus en plus sur un plus grand nombre d'objets. Sont intervenus ensuite :
le lavabo, d'autres jeux, le buffet.
Voilà comment ensuite, elle commente ces quelques indications d'analyse qu'elle nous donne,
et comment elle comprend le cas de DICK.
Elle répète que ce qui a fait qu'il se défende plus fortement qu'un autre contre son sadisme était son développement génital précoce.
Et là elle donne comme signes de ce développement génital précoce le fait que quand l'enfant venait
par exemple de vouloir manger un petit jouet,
un petit personnage-jouet, qui repré¬sentait le pénis du père, après l'avoir fait, il montrait des sentiments de pitié pour le personnage ainsi lésé,
et s'apitoyait, et voulait restituer ce qu'il avait voulu lui prendre et posséder.
Sa capacité précoce de sympathie, de sentir ce que sent l'autre, c'est ce qu'elle donne comme signe
de développement génital précoce.
De ce fait-là, il a retiré son intérêt de tout
le corps extérieur, qui était le corps, le pénis,
et tout cela est devenu dangereux, agressif.
Il s'est coupé de la réalité, et le développement
des fantasmes s'est arrêté.
Cela me semble extraordinaire.
Il se réfugie dans un seul fantasme…
dans le fantasme du corps vide
et sombre, noir, de la mère
…et il retire son intention des objets extérieurs
qui représentaient le contenu du corps de la mère.
Voilà comment elle comprend l'action analytique pour DICK.
Elle pense que les interprétations qu'elle a données…
elle souligne un point de technique en disant
que d'habitude elle n'interprète jamais sur un matériel aussi unique. Elle attend d'avoir le matériel. Mais là comme il n'y avait qu'une sorte de matériel, elle a agi tout de suite
…elle pense que d'avoir résolu ou atteint le problème inconscient a entraîné une sédation de l'anxiété.
Cette détente de l'anxiété a permis un nouvel afflux d'anxiété, et ainsi, chaque fois, à chaque nouvelle interprétation, et :
- que chaque anxiété, ainsi tombée après l'interpréta¬tion, se distribuait, comment elle l'avait décrit au départ, sur les objets exté¬rieurs,
- que d'ailleurs on le voit bien, parce que dans
le traitement, comme dans la vie courante, il avait contact et s'intéressait à un nombre d'êtres qui allait croissant.
Elle décrit ses progrès généraux :
- il peut être plus agressif,
- s'intéresse à des objets de plus en plus nombreux,
- le transfert avec elle est plus fort,
- le vocabu¬laire augmente : elle dit qu'il apprend des mots nouveaux et qu'il se souvient de davantage de mots,
- il devient plus affectueux avec sa mère et avec son père. Elle ne précise pas, mais son attitude se modifie : il semble aborder une phase œdipienne.
Elle fait une sorte de conclusion de la compréhension théorique de tout cela, en répétant que pouvoir maîtriser l'anxiété mieux, c'est pouvoir la dis¬tribuer sur des objets et des intérêts plus nombreux, et qu'ainsi les quantités d'anxiété sont régularisées, l'ego devient de ce fait capable de les tolérer et de les maîtriser.
Si l'on veut résumer, on peut dire :
- que l'enfant part d'un stade initial où domi¬nent les mécanismes d'introjection et l'expulsion, un stade d'identification généralisée,
- et qu'il doit passer à un stade de formation symbolique, dont l'iden¬tification est une sorte de précurseur.
Des identifications symboliques seraient à la base de toutes les sublimations et activités ultérieures.
Et que cette instau¬ration du stade symbolique permettra
le libre jeu de la fantaisie, du fantasme, le passage entre les deux se fait par la distribution de l'anxiété, qui investit des objets du monde extérieur de plus en plus nombreux.
J'ai laissé tomber la fin du chapitre sur le diagnostic.

Je vais donner mes réactions personnelles sur ce texte. Je me suis centrée autour de la notion de symbolisme, qui m'apparaissait capi¬tale, tout entière posée là-dedans.
Mais avant je dois dire qu'on pourrait faire sur presque chacun des paragraphes de ce texte des remarques qui ont rendu pour moi la compréhension difficile.
Comme par exemple celles-ci :
des choses qui me semblent contradictoires tout le temps, j'en relèverai une ou deux.

Par exemple, elle dit que l'enfant retirait son intérêt du monde extérieur, des objets, qu'il niait ce monde extérieur, parce que trop dan¬gereux.
Et elle dit : « Donc il n'y a pas de relation. »
Cela me semble contradictoire.
S'il nie quelque chose, c'est qu'il nie une relation, et que cette relation, donc ces objets, sont investis de quelque chose qu'il cherche à nier.
S'il nie, c'est qu'il y a quelque chose.
Pour moi, il semble, il faudrait, on ne peut pas être satisfait par sa description clinique au départ, disant que DICK n'a de relation avec rien.
Il me semble qu'il y a des relations, mais qu'il les nie, parce que ces relations sont dangereuses.
Aussi le passage où elle dit qu'il se réfugie dans le fantasme du corps vide et noir de sa mère.
Et, dit-elle :
« Il retire son intérêt des objets qui représenteraient
le contenu de ce corps enfants, excréments, pénis… »
Je ne peux pas comprendre comment un fantasme pourrait être morcelé comme ça, comment l'enfant pourrait être rassuré à l'intérieur du corps et que cet inté¬rieur du corps n'implique pas tout le reste ; il faut qu'il soit quelque part.
Peut-¬on prendre un petit morceau de quelque chose ? Là, je ne peux pas comprendre.
Une troisième chose sur laquelle… qui m'a paru tout à fait curieuse :
com¬ment elle comprend le fait que DICK récupère du vocabulaire.
Elle a l'air de pen¬ser qu'à partir du commencement
où elle le soigne il apprend des mots, comme s'il se mettait à avoir de la mémoire et à désirer apprendre.
Cela m'a frappée, parce que j'ai eu l'occasion de faire l'investigation d'un enfant :
- il avait l'âge de DICK, cinq ans,
- il avait exactement le même tableau clinique,
- il n'y a aucune dif¬férence particulière, toutes ces histoires de vocabulaire étaient semblables.
Et à partir du moment où il s'est mis à parler, c'était très clair : dans ce cas-là, il n'apprenait pas des mots, il les savait, mais il ne niait plus de les savoir.
Autrement dit, il avait tout un acquis de syntaxe et de vocabulaire mais il les niait, de même qu'il niait ses relations avec les objets extérieurs. Et alors elle pense qu'il s'est mis à apprendre des mots.
J'arrive - pour terminer - à sa notion, à la critique de sa notion de symbolisme.
Si j'ai bien compris, il semble que pour elle, donc, le symbolisme soit un pro¬cédé très simple, unilinéaire, sans dessus ni sans dessous, qui est une équation entre le corps, total ou partiel, et des objets du monde extérieur. Alors il se pose des questions.
C'est qu'en décrivant cela elle note très bien…
et elle y insiste beaucoup
…la corrélation dans le temps entre l'avènement
du symbolisme, le moment où il y a symbolisme, c'est quand l'enfant fait le renforcement de l'ego.
Mais elle n'explique pas la liaison.
Elle ne la rend pas compréhensible : elle montre que ça va à peu près ensemble, et la fonction de l'ego n'est pas du tout précisée, ni même abordée.
Il semble qu'elle considère, elle, le symbolisme comme un mécanisme, parmi d'autres, d'adaptation.
Bien sûr, elle dit qu'il est fondamental, et que sans lui rien ne pourrait aller, parce que, DICK n'ayant pas acquis ce mécanisme, il est stoppé.
Mais pour elle ce mécanisme n'est qu'un mécanisme parmi d'autres, qui ne change pas la structure de l'ensemble du sujet, mais rend plus facile et plus pos¬sible la tâche de l'ego qui est de maîtriser l'anxiété et s'adapter.
Et enfin elle ne rend pas du tout compte comment cette distribution d'anxiété sur les objets,
c'est-à-dire comment de ce monde fantasmatique
on passe à des relations avec un vrai réel, un réel réel.
Elle dit qu'un ego plus fort peut le faire. Mais comment ?
Et qu'est-ce que c'est ce réel, elle ne le dit pas du tout.
Alors, voilà comment il semble que je formulerais
ces problèmes pour me les rendre cohérents :
c'est de considérer le premier stade d'où elle part, le stade de la phase sadique-orale, dont elle décrit surtout les contenus.
Si on considère que ce stade, surtout du point de vue de sa structure, de son organisation, on peut peut-être dire que tout y est à la fois fantasmatique et réel, dans ce sens qu'il n'y a encore ni réel ni fantasmatique vrai, puisque rien ne le différencie.
C'est un pré-¬réel et un pré-fantasmatique.
LACAN - D'ailleurs elle le dit, elle le formule :
« unreal reality ».

Il semble que ce stade est caractérisé par une indifférencia¬tion du tout, et que le sujet n'y est rien d'autre que ce double mouvement d'introjection et d'expulsion qui est un cercle vicieux, et dont tous les moments sont angoissants, que ce soit
le moment d'introjection, ou le moment d'expul¬sion : par introjection du sadisme, et expulsion de ce qui est bon.

Mais c'est plutôt ce qui est mauvais qui est expulsé.

Oui. Mais il expulse tout à la fois.
Je dois dire que l'anticipation d'une structure nouvelle se présente toujours d'abord sous la négativité avant de passer à une autre forme.
Et dans ce cas la négativité est le sadisme.
Mais tant que cette négativité demeure dans ce registre d'indifférenciation, où le réel et le fantasmatique sont confondus, dans quel ego ressent-il le quasi-réel ?
On reste toujours dans le cercle vicieux, parce que cette négativité ne peut pas aller à son terme où elle serait structurante et ferait pas¬ser à un autre stade de différenciation, où s'effectuerait
la différenciation du fan¬tasmatique et du réel.
Dans le cas de DICK, il semble que ce passage se soit fait par l'intervention de l'analyste, qui intervient ici comme un troisième terme, qui dans ce cas com¬prend le fantasme, le valorise, permet le sadisme, parce que par la verbalisation il différencie le symbolique du réel :
il rend symbolique le corps de la mère et rend symbolique le sadisme.

Elle lui fout le symbolisme avec la dernière brutalité.
Elle com¬mence tout de suite par lui flanquer les interprétations majeures, elle le flanque dans une verbalisation absolument brutale…
et presque aussi révoltante pour nous
que pour n'importe quel lecteur
…de ce que c'est que le mythe œdipien :
« Tu es le petit train, tu veux foutre ta mère. »
C'est sa méthode!
Évidemment, cela prête à toutes sortes de discussions théoriques, qui ne peu¬vent pas être dissociées du diagnostic du cas.
Mais il est certain qu'à la suite de cette intervention
il se produit quelque chose, et tout est là!

Ce que vous expliquez, le manque de contact,
c'est le défaut de l'ego.
Ce sujet a, à proprement parler, un ego qui n'est pas formé.
Et la façon dont elle distingue justement…
jusque dans la profonde indifférence,
apathie, absence, de ce sujet au début
…dont elle le distingue, dont elle le tranche avec les autres névrosés, c'était déjà assez significatif.
Mais ce qui est tout à fait clair, c'est en somme quoi ?
Ce qui correspond à ce que je vous ai dit :
ce qui n'est pas sym¬bolisé, c'est la réalité,
avant toute formation de symbole.
On peut dire que ce jeune sujet, c'est ça qui le limite. Il est tout entier dans la réalité à l'état pur, inconstituée : ce qui n'est pas différencié.
C'est ce qu'elle nous montre, c'est l'in¬térêt pour les objets en tant que distincts, en tant qu'objets de l'intérêt humain, en grand nombre, en objets équivalents, c'est le développement de ce monde infini d'objets qui constitue un monde humain.
Et ce qu'elle nous indique, ce en quoi son texte est précieux, parce qu'elle est ce qu'on pourrait appeler quel¬qu'un, une thérapeute, une femme d'expérience, elle sent ces choses, elle les exprime mal, on ne peut pas le lui reprocher.
En tout cas, en vérité, la théorie de l'ego est incomplète, elle n'est peut-être pas décidée à
la donner, et c'est ce qui manque.
Mais ce qu'elle explique très bien, c'est ceci…
je suis forcé d'aller vite aujour¬d'hui,
et je reprendrai la prochaine fois
…ce qu'elle montre est ceci :
que si les objets du monde humain se démultiplient, se développent, avec la richesse qui constitue l'originalité de ce monde, c'est dans la mesure où, dans une sorte de processus d'expulsion, lié à l'instinct
de destruction primitif, ces objets…
dans leur première signification que nous appellerons, si vous voulez « affective »…
pour aller vite aujourd'hui, puisque
nous avons un peu exorcisé ce mot et compris ce qu'il veut dire
…c'est d'une certaine relation primitive à la racine même instinc¬tuelle de l'être qu'il s'agit
…dans la mesure où ces objets se créent, apparaissent, par un processus d'expulsion et de destruction, et également
qu'à mesure que se produisent ces éjections par rapport au monde du sujet primitif, non encore organisé dans le registre de la réalité proprement humaine, communicable, que surgit à chaque fois - quoi ? –
un type d'identification qui, disons, n'est pas sup¬portable.
L'anxiété n'est pas une espèce d'énergie que le sujet aurait à répartir pour constituer les objets…
d'ailleurs il n'y a aucune tournure de phrase,
à pro¬prement parler, dans ce sens
…l'anxiété est toujours définie comme surgissant, « arising ».
À chacun de ces rapports objectaux correspond un mode d'identification dont l'anxiété est à proprement parler le signal.
Chaque fois que le sujet s'iden¬tifie avec quoi que ce soit…
et ici c'est quelque chose qui précède l'identification qui est à proprement parler moïque,
même quand l'identification moïque sera faite
…toute nouvelle réidentification du sujet,
fera surgir, l'anxiété, anxiété au sens où anxiété est tentation, vertige, re-perte du sujet pour se retrouver à ces niveaux extrêmement primitifs.
L'anxiété est telle qu'effectivement il ne peut pas se produire, ce jeu où le sujet introduit pour chaque objet d'anxiété, mais ce n'est qu'une connotation.
L’anxiété c'est un signal, comme FREUD a toujours… lorsqu'il a approché la notion d'anxiété
…très bien senti et formulé, que c'était une sorte
de signal, une qualité, une coloration subjective.
Cette anxiété c'est précisément ce qui en somme ne
se produit pas, car le sujet ne peut même pas arriver à cette sorte d'identification qui serait déjà une ébauche de symbolisme.
Il reste en face de la réalité, ce sujet, si paradoxal que ce soit, il vit dans la réalité :
- il est dans le bureau de Mélanie KLEIN,
- il n'y a pas pour lui d'autre,
- il n'y a pas pour lui de lui-même,
- il y a une réalité pure et simple : l'inte
